Prior to establishing Mötley Crüe, Sixx was a member of Sister, following which he formed London alongside his Sister bandmate Lizzie Grey. Additionally, in 2000, he established a side project group called 58, comprising of Dave Darling, Steve Gibb, and Bucket Baker and released their album, Diet for a New America.
In 2002, Sixx formed the hard rock supergroup Brides of Destruction, featuring L.A. Guns guitarist Tracii Guns. In 2006, Sixx formed Sixx:A.M., initially to record an audio accompaniment to his autobiography, The Heroin Diaries: A Year in the Life of a Shattered Rock Star, which featured songwriter, producer, and vocalist James Michael and guitarist DJ Ashba.

In 2006, Sixx, alongside Kelly Gray, formerly the co-president and house model of St. John, launched the clothing line Royal Underground. Initially, the label focused on men's clothing before expanding into women's apparel.
Additionally, in 2010, Sixx and Jenn Marino co-hosted the nationally syndicated rock/alternative music radio programs Sixx Sense and The Side Show Countdown, launched by Premiere Radio Networks and based in Dallas, Texas.
