Cantrell's unique vocal style and harmonization with Layne Staley (and later William DuVall) contributed to the band's international success.

In 1992, Cantrell began singing lead vocals on Alice in Chains' EP Sap. Following Staley's death in 2002, Cantrell took on the role of the band's lead singer in most of the songs from their post-Staley albums, including Black Gives Way to Blue (2009), The Devil Put Dinosaurs Here (2013), and Rainier Fog (2018). In live concerts, DuVall harmonizes with Cantrell in new songs and sings Staley's vocals in old songs.

Aside from his work with Alice in Chains, Cantrell has also pursued a solo career. He released the albums Boggy Depot in 1998 and Degradation Trip Volumes 1 & 2 in 2002, with his third solo album, Brighten, released in 2021.
